Instructions have been updated this year, please read carefully.

The COA/Municipality shall make no changes to the contract!

*Due to the high volume of contracts, scanned copies will not be accepted.

The contract section entitled “BRIEF DESCRIPTION OF CONTRACT PERFORMANCE or REASON FOR AMENDMENT” provides a description of the terms of the Contract the COA/Municipality is bound. In addition, incorporated into the contract are the Terms and Conditions uploaded with the Contract.   • The address in the upper left hand corner box of the contract may not be the COA’s address; it is the current legal address of the municipality on file with the Office of the State Comptroller.

  • The Executive Office of Elder Affairs countersigns in the lower right field corner; do not mark or make any changes in that area.
  1. The authorized signatory shall sign and date this contract as an original, PLEASE note the date cannot be typed. The COA director or COA board chair should print the signatory’s name and title in the field located at the bottom left section of the form. Only the chair of the select board, executive secretary, mayor or someone legally authorized to sign contracts, may sign on behalf of the municipality.

  2. An original Contractor Authorization Signatory Listing (CASL) form is expected. Please disregard the note referring to page 2 as being optional. Page 2 is REQUIRED. Send the original CASL form with your executed contract.

  3. If the COA requests an original signed copy of the contract, two original signed signature pages are to be returned to EOEA with a self-addressed envelope. One signed copy of the Contract with original signatures will be returned to the COA/Municipality.

  4. Changes made to the contract will not be accepted. Please include a memo advising ELD of any updates you wish to include going forward and enclose the memo when mailing in your contract.

  5. As noted in the Brief Description of the Contract Performance or Reason for Amendment field, upon receipt of the properly signed contract, EOEA will issue a payment in the amount noted in the Compensation Field.

  6. Please submit as soon as practical. You need NOT overnight the contract. Return the document to:

Notice of Intent to Award Formula Grants to Local Councils On Aging In SFY21

The Massachusetts Executive Office of Elder Affairs (EOEA) announces its intent to award Formula Grants to local Councils on Aging in State Fiscal Year 2021. Established pursuant to M.G.L. 19A § 1, EOEA promotes the independence, empowerment, and well-being of older people, individuals with disabilities and caregivers throughout the Commonwealth of Massachusetts.
 
Councils on Aging (COAs) are municipally appointed agencies that serve as the local gateway to EOEA and other state and federal programs and services. The 350 COAs in Massachusetts annually provide direct services to more than 330,000 elders, their families and caregivers. While each COA is unique, most Councils offer information and referral, transportation, outreach, meals (congregate and home delivered), health screening, fitness and recreational programs.
 
Formula grant funding is authorized through Chapter 227 of the Acts of 2020, which was signed into law on December 11, 2020. Awards will be issued to each COA at a rate of $12.00 per elder, based upon the number of elders noted in the 2010 federal census. Regardless of size, all COAs are eligible to receive a minimum award of $6,000.
 
In FY2021, Service Incentive Grants are also to be awarded and issued to local Councils on Aging who have demonstrated and identified unmet needs for sub-populations whose needs are unique and are not readily services through typical state and local funding; and, to the Massachusetts Councils on Aging (MCOA) for statewide service priorities.

The COA/MUNICIPALITY SHALL MAKE NO CHANGES WHATSOEVER TO THIS CONTRACT! See instructions for the authorized municipal signatory in item #1, below.

The Formula Grant payment is noted at “COMPENSATION.” The FY2020 Formula Grant award is the sum of the “COMPENSATION” figure less any carryover balance recorded on your Statement of Grant/Allocation Balance for FY2019.  If your FY2020 Formula contract does not appear on the website, please contact the COA Program Manager at Elder Affairs 1-800-698-9723.

The contract section entitled “BRIEF DESCRIPTION OF CONTRACT PERFORMANCE or REASON FOR AMENDMENT” binds you to the contract and “The Green Guide” to COA Eligible Costs and Best Practices for FY2020 issued with the Formula Grant/Allocation notice. If you do not have a current copy of “The Green Guide” please contact Elder Affairs COA Program Manager.

  2. Please contact ELD prior to making any contract modification! If you already changed the name of the contract manager (upper left hand box), have the signatory initial that change!

  3. Please submit as soon as practicable. You need NOT overnight the contract.
